Summary: We are looking for a compassionate and organized Case Manager. The case manager helps identify appropriate providers and services while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for the client. The case manager works primarily with clients who are low-income and in crisis or emergency situations. Case Managers are Mandated Reporters while working with the Family Support division. 

Role Assignment: This is a full-time, grant funded position located at the Family Services Goshen Family Center site. The Family Center is a mobile unit located on the property of Goshen Elementary School. The Case Manager will predominately manage cases of families with children 0-5 but is also able to assist families from the community whose children do not fall into this age range. The Case Manager will conduct home visits in Goshen as needed, to assess the needs for services and make referrals to supporting agencies. The Case Manager will administer intakes which may include ACES screenings and Protective Factors Screenings. The Case Manager will work with the In-Home Parenting (IHP) team to assist, plan and attend community events, food distributions, and diaper distributions.

Primary Responsibilities:

  • Conducts phone intake assessments and in-person appointments. Manages documentation for organizational purposes, as well as management and reporting for federal and non-federal grants.
  • Plan, schedule, and teach one on one parenting classes and workshops.
  • Plan, schedule, and facilitate creatively themed support groups.
  • Counsels clients and/or their families to facilitate achieving service plan goals, developing life skills, and/or sustaining financial stability, conducts activities, and monitors the service environment in a manner to maximize client success and well-being.
  • Composes or prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications.

Other Responsibilities:

  • Works with community partners to maximize assistance for clients.
  • Advocates on behalf of clients and families for services, basic needs, and other related issues.
  • While working in this capacity, you will be a Mandated Reporter. You will receive training and guidance to further understand this responsibly.
  • Conduct comprehensive, client-centered social work activities in accordance with best practices.
  • Must demonstrate compassion, especially when clients are experiencing complex issues or in crisis.
  • Stay up-to-date on community resources and procedures for referring clients to appropriate resources.
  • Prepare required quarterly and annual reports.
  • Order and maintain supplies via inventor of vouchers and other products or services approved for distribution.
  • May refer clients to other emergency assistance providers.
  • Perform all tasks in a safe manner to avoid injury to self and others.
  • Adherence to policies related to HIPAA, confidentiality, information sharing, and appropriate use of the agency-client database.
  • Performs other duties and assumes other responsibilities as assigned by the supervisor or Leadership Team.

Qualifications:

Education: Bachelor’s degree from an accredited college or university with emphasis in social work or 1 year of working/volunteer direct service experience and an Associate’s degree in social work or similar field. Two years of case management or working/volunteer direct service experience may be accepted in lieu of a degree.

Experience: One year of client services or case management, preferably in a social service or health-related agency. Ability to communicate effectively with a diverse audience. Knowledge of general interviewing practices and/or techniques; knowledge of social work and counseling practices and techniques, including crisis prevention and intervention. Must have experience with Microsoft Office Suite & ability to work on online platforms. Must demonstrate compassion, especially when clients are experiencing complex issues or in crisis. Must demonstrate cultural competence, to understand and value clients’ unique values and perspectives.

Transportation: California Driver’s License, availability of automobile for business use, and at least minimum auto liability insurance as required by law. This will require some travel throughout Tulare County as needed.

Additional Requirement: Must receive clearance from the Department of Justice Criminal Record Offender Information database. Must be at least 21 years of age.

Physical Demands & Working Environment:

Environment: Standard office and client residence setting; occasional travel from site to site; occasional exposure to the sun, moderate heat, noise, dust, and fumes; stressful calls/situations. Requires the kind of teamwork, supervision, and personal interaction that cannot be had in a home office situation; therefore, regular and predictable on-site attendance is a job requirement.

Physical: Primary functions require sufficient physical ability to work in an office setting and field environment including occasional travel to various locations; sit for prolonged periods; stand for brief periods; climb 3-5 stairs; operate modern office equipment. Work requires lifting, stooping, bending, stretching, walking, standing, pushing, pulling, reaching, and other physical exertion This position requires the ability to lift, bend, and carry items of 25 pounds when setting up events and breaking down pallets of food/diapers including long periods of standing and exposure to the outside elements during winter and summer.

Vision: See in the normal visual range with or without correction; vision sufficient to read computer screens and printed documents and to operate the equipment.

Hearing: Hear in the normal audio range with or without correction.

REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Demonstrate awareness and competency in providing services to multi-ethnic, multi-cultural groups and individuals.
