Job Summary: We are seeking a compassionate and organized part-time Case Manager to work primarily with low-income clients in crisis or emergencies. This is a grant-funded position. The Case Manager will identify appropriate providers and services, ensuring resources are used effectively to benefit the client.
Role Assignment: The Case Manager will be involved with various programs in the Supportive Housing Department, focusing on families and individuals seeking Cal AIM services in Tulare County, as well as assisting with the Transitional and Permanent Housing programs. Responsibilities include client support, documentation, intakes, enrollments, case management, home visits, record keeping, and referrals to partner agencies. The role requires occasional travel within and outside Tulare County.
Primary Responsibilities:
· Intake Assessments and Documentation: Conduct phone and in-person intake assessments to understand clients' needs. Manage documentation for organizational purposes and grant reporting, ensuring accuracy and completeness.
- Peer Counseling and Support: Provide peer counseling to clients and their families to help them achieve their service plan goals. Assist in developing life skills and maintaining financial stability. Facilitate life skill classes and monitor the service environment to support client success.
- Documentation and Reporting: Prepare correspondence, case notes, reports, and other necessary documentation using computer-based applications. Ensure all records are maintained accurately and up-to-date.
- Community Collaboration: Work closely with community partners to enhance the support and resources available to clients. Foster collaborative relationships to improve client assistance.
- Client Advocacy and Financial Coordination: Advocate for clients’ needs and work directly with the finance department to manage service payments. Ensure clients receive the support and resources they require.
Other Responsibilities:
· Communication Management: Check and respond to all forms of communication, including voicemail, email, Teams, and text, within 24 hours to ensure timely and effective responses.
· Client-Centered Social Work: Conduct social work activities in a client-centered manner, following best practices to support clients effectively.
· Resource Awareness and Referrals: Stay informed about community resources and referral procedures. Provide clients with appropriate referrals to meet their needs.
· Report Preparation: Prepare required quarterly and annual reports to document program activities, client progress, and outcomes.
· Supply Management: Order and maintain necessary supplies. Perform additional tasks as assigned by the supervisor or Leadership Team to support program operations.
Qualifications:
· Education: Bachelor’s degree in social work or related field, or 1 year of direct service experience with an Associate’s degree in social work. Two years of case management experience may be accepted in lieu of a degree. Bi-lingual skills are preferred.
· Experience: At least one year of client services or case management experience, preferably in a social service or health-related agency. Familiarity with Microsoft Office Suite and online platforms is required.
· Transportation: Valid California Driver’s License, availability of an automobile for business use, and minimum auto liability insurance.
· Additional Requirement: Clearance from the Department of Justice Criminal Record Offender Information database and must be at least 21 years of age.
Physical Demands & Working Environment:
· Environment: Standard office and client residence settings; occasional travel and exposure to sun, heat, noise, dust, and fumes. Regular on-site attendance is required.
· Physical: Ability to work in an office and field environment, including sitting, standing, climbing stairs, lifting up to 25 lbs, and performing other physical tasks.
· Vision & Hearing: Normal visual and auditory range with or without correction.
· Reasoning Ability: Ability to solve practical problems and interpret various instructions. Demonstrate cultural competence and awareness in providing services to diverse groups.
