Summary: We are looking for a compassionate and organized Case Manager. The case manager is a grant-funded position that works primarily with low-income clients and in crisis or emergencies. The case manager helps identify appropriate providers and services while ensuring that available resources are being used in a timely and cost-effective manner to obtain optimum value for the client.
Role Assignment This assignment will involve working with Permanent Supportive Housing Clients in Tulare County. The Case Manager will provide support and guidance, including working with clients to gather required documentation, conduct intakes, handle enrollments, manage cases, perform home visits, and maintain records. Additionally, the role includes making resource referrals to partner agencies and other necessary services. This position will require some travel throughout Tulare County and occasionally outside Tulare County as needed.
Primary Responsibilities:
- Conduct phone intake assessments and in-person appointments. Manages documentation for organizational purposes, as well as management and reporting for federal and non-federal grants.
- Counsels clients and/or their families to facilitate achieving service plan goals, developing life skills, and/or sustaining financial stability, conducts activities, facilitates life skill classes, and monitors the service environment in a manner to maximize client success and well-being.
- Assist clients in gathering required documentation for housing and services.
- Ensure all necessary forms and documents are completed accurately and promptly.
· Advocates on behalf of clients and families for services, basic needs, and other related issues.
- Conduct regular home visits to monitor client progress and provide support.
- Assist clients in navigating complex service systems and addressing barriers to housing stability.
- Composes or prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications.
· Works with community partners to maximize assistance for clients.
· Check all forms of communication daily including voice mail, email, Teams, and text, and respond within 24 hours.

Other Responsibilities:
- Works directly with the finance department for proper management and payment of services to landlords and utility companies.
- Must demonstrate compassion, especially when clients are experiencing complex issues or in crisis.
- Stay up-to-date on community resources and procedures for referring clients to appropriate resources.
· Order and maintain supplies via inventor of vouchers and other products or services approved for distribution.
· Participate in ongoing training and professional development opportunities.
· Stay informed about current trends and developments in the field of Transitional Housing and case management.
· May refer clients to other emergency assistance providers.
· safely perform all tasks to avoid injury to self and others.
· Adherence to policies related to HIPAA, confidentiality, information sharing, and appropriate use of the agency-client database.
- Performs other duties and assumes other responsibilities as assigned by the supervisor or Leadership Team. These duties are subject to change based on the needs of the program and organization.
Qualifications:
- Education: Associate's degree from an accredited college or university with emphasis in social work, psychology, sociology, human services, or a related field. Two years of case management or working/volunteer direct service experience may be accepted in place of a degree.
- Experience: One year of client services or case management, preferably in a social service or health-related agency. Experience working with homeless populations or in a similar social services role. Ability to communicate effectively with a diverse audience. Knowledge of community resources and social services. Knowledge of general interviewing practices and/or techniques; knowledge of social work and counseling practices and techniques, including crisis prevention and intervention. Must have experience with Microsoft Office Suite & ability to work on online platforms. Must demonstrate compassion, especially when clients are experiencing complex issues or in crisis. Must demonstrate cultural competence, to understand and value clients’ unique values and perspectives.
- Transportation: California Driver’s License, availability of automobile for business use, and at least minimum auto liability insurance as required by law.
- Additional Requirement: Must receive clearance from the Department of Justice Criminal Record Offender Information database. Must be at least 21 years of age.
· Physical Demands & Working Environment:
· Environment: Standard office and client residence setting; weekly travel from site to site; occasional exposure to the sun, moderate heat, noise, dust, and fumes; stressful calls/situations. Requires the kind of teamwork, supervision, and personal interaction, that cannot be had in a home office situation; therefore, regular and predictable on-site attendance is a job requirement.
· Physical: Primary functions require sufficient physical ability to work in an office setting and field environment including travel to various locations; sitting for prolonged periods; standing for brief periods; climbing stairs; operate modern office equipment. Work requires lifting, stooping, bending, stretching, walking, standing, pushing, pulling, reaching, and other physical exertion. Lift to 25 lbs.
· Vision: See in the normal visual range with or without correction; vision sufficient to read computer screens and printed documents and to operate the equipment.
· Hearing: Hear in the normal audio range with or without correction.
· REASONING ABILITY:
o Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
o Ability to adapt to the situation at hand.
o Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
o Demonstrate awareness and competency in providing services to multi-ethnic, multi-cultural groups and individuals.
